Required Documentation

Below is a list of what must be included with your application as supporting documentation.

  • Illness/Injury – Provide physician documentation that identifies the type of illness or injury and the length of time that the individual is unable to work.
    • For out-of-the ordinary medical costs, please include physician statement that indicates cost required to receive treatment.
    • For consideration of medical expenses provide physician documentation with medical necessity for treatment and cost associated with treatment
  • Disaster – Copies of fire or insurance reports
  • Domestic Violence – Include a copy of an active restraining order & bills that must be paid
  • Death of a loved one – Include the deceased name, as well as the name, city and state of the funeral home
  • Utility bills are not necessary or required
