What do I need?
Following are the current guidelines for hardship assistance:
Disaster Assistance
Levels of emergency assistance in cases related to a disaster are based on household family size, household income and level of damage. This type of assistance is limited to one case per family per incident. Assistance received from relief organizations and/or insurance will be considered when reviewing this type of request and determining the grant amount. Below is the funding grid utilized when providing assistance in a disaster related situation. No grant for disaster shall exceed $2,500 per family.
DOCUMENTATION REQUIRED: Copies of fire or insurance reports.
Extended Illness/ Injury
- A request will be considered for hardship assistance in cases related to extended illness or injury of the eligible recipient or his or her immediate family, which requires an absence from work and results in critical, short-term economic hardship.
- A request will be considered when the applicant or family member requires treatment with non-routine medical expenses (e.g. required out of network treatment, long distance travel to receive treatment).
- A request will be considered when the cost of medical treatment is preventing an applicant or dependent family member from receiving care.
- A request will be considered when the applicant has incurred extreme medical bills and the household income is 200% below poverty level.
- No grant for illness/injury shall exceed $1,500 per family.
DOCUMENTATION REQUIRED
- Provide Physician documentation that identifies the type of illness or injury and the length of time that the individual is unable to work.
- For out-of-the ordinary medical costs, please include physician statement that indicates cost required to receive treatment and medical necessity.
- For consideration of medical expenses provide physician statement of medical necessity, bills for treatment and household income.
Death
- A request will be considered when death in the immediate family is causing financial hardship. Immediate family includes employee’s legal dependents, parent or legal guardian, and step-parent.
- A request will be considered when death or terminal illness in immediate family is requiring long-distance travel. Funding will only be provided once, for either travel to help care for terminally ill family member OR travel for funeral. Immediate family includes the employee’s parent or legal guardian, stepparent, brother, sister, current spouse/domestic partner, children, stepchildren, grandparents, grandchildren, current mother-in-law and current father-in-law.
- A request will be considered for assistance with death of an employee.
DOCUMENTATION REQUIRED:
- Include individual’s name, relationship to applicant, as well as the name, city and state of the funeral home.
- No grant for other assistance shall exceed $1000 per family per incident.
Domestic Violence
- A request will be considered when domestic violence has caused a financial hardship.
DOCUMENTATION REQUIRED:
- Include an active restraining order and bills that must be paid.
- No grant for other assistance shall exceed $1000 per family per incident.
UTILITY BILLS ARE NOT NECESSARY OR REQUIRED
