What’s Your Role?
As a Grant Leader, you are the go-to person when people at your facility have questions about the Hope Fund. The fund basics are outlined below.
As a facility HCA Hope Grant Leader, you:
- Receive application
- Verifies employment status meets eligibility
- Ensures application is complete (if not, requests additional information)
- Determines number of unpaid hours
- Schedules committee meeting (within 48 hours of receiving application)
- Facilitates committee review meeting
- Completes committee recommendation form based off of committee review
- Submits all (both approved and declined) applications including supporting documents and committee recommendation form to Hope Fund staff via fax.
- Once Hope Fund staff reviews application and recommendation form for consistency and guideline eligibility, Grant Leader notifies applicant of final decision.
- Provides Hope Fund staff with direction for payment processing (how to provide funds: direct deposit, check to applicant, check to creditor), in some cases Grant Leader will present the check to the applicant
- Receives notification letter (both with approval and declination), attaches to the original application and files
- Maintains files for Hope Fund applicants
- Receives and communicates updates/clarification to Hope Fund committee
- Provides valuable feedback (including guideline recommendations to Hope Fund staff as received and/or as requested
